About the COVID-19 Vaccination Program
The COVID-19 vaccination program has started in Koriyama. The vaccine will be made available to all registered residents of Koriyama, regardless of nationality, and will be free of charge.
Vaccination Coupons
All registered residents of Koriyama City will received (or have already received) a Vaccination Coupon.
The contents of this envelope are very important, so do not throw them away.

The envelope for your third vaccination (booster) includes a 'Certificate of Vaccination for COVID-19'. This lists which vaccinations you have received so far, and when. Use your Vaccination Ticket Number (あなたの接種券番号) when reserving your vaccination. |

Pre-Vaccination Screening Questionnaire

A Pre-Vaccination Screening Questionnaire will be sent along with the Vaccination Coupon. For the third vaccination, the questionnaire is attached to your Certificate of Vaccination for COVID-19. This questionnaire is about health condition of, and is very important to determine if the vaccination can be done safely. Please fill in the questionnaire before going to the vaccination venue or clinic. How to make a reservation
Once you receive your vaccine ticket, you can make a reservation. How reservations can be made differs per clinic or location. At some locations, you can only reserve by phone. Others allow you to book an appointment online.
You can use the two websites below to find a location and book an appointment:
1) COVID-19 Vaccine Navi

Use the Ministry of Health, Labour and Welfare's COVID-19 Vaccine Navi website to find all clinics in the city offering COVID-19 vaccination. Note: It includes clinics that do not allow online reservations, but only by phone. |

2) COVID-19 Vaccine Web Reservation (Koriyama City)

Koriyama City's COVID-19 Vaccine Web Reservation system lets you log in with you Vaccination Number, and lists all the clinics where you can book an appointment online. Select your preferred date and time. |

What to bring when receiving your vaccination
- Vaccination Coupon (接種券, sesshuken)
- Proof of Identification, such as:
- Residence Card
