When you become pregnant
When you find out that you are pregnant, you must submit a "Pregnancy Notification" at City Hall.
What to bring:
- Pregnancy Notification (This is available at your hospital or the contact shown below.)
- Something that shows your My Number
(Click here for information on your My Number) - Proof of identity (passport, residence card, My Number card, etc.)
Where to submit:
- Koriyama City Chuo Public Health Center (located inside the Koriyama City Public Health Center)
- Koriyama City Minami Public Health Center (located inside the Koriyama City Hall Asaka Branch Office)
- Koriyama City Kita Public Health Center (located inside the Koriyama City Hall Fukuyama Branch Office)
- Koriyama City Nishi Public Health Center (located inside the Koriyama City Hall Katahira Branch Office)
- Child and Family Support Division (located inside Niko Niko Kodomo Kan)
At these locations, you will receive your "Maternal and Child Health Handbook" and "Mother and Child Health Leaflet."
Your free Maternal and Child Health Handbook is written in Japanese paired with one of the following languages: English, Chinese, Korean, Tagalog, or Vietnamese.
What is the Maternal and Child Health Handbook for?
This is an important handbook for keeping records of the mother's prenatal period and the health of her child. It is also used to keep records of your child’s immunizations. Make sure to always bring this with you on your hospital visits. You will be using this Handbook even if you move to another municipality. Please do not throw it away.
What is the Mother and Child Health Leaflet for?
You can receive periodic health checkups at the hospital. This leaflet is necessary to receive subsidies for the cost of your prenatal and postnatal health checkups, as well as for the hearing test for your newborn baby. Make sure to bring your Maternal and Child Health Handbook, Mother and Child Health Leaflet and personal identification with you whenever you visit the hospital for a health checkup during your pregnancy.
